Annotation | The National Service Framework for Older People (NSF) envisages that the Single Assessment Process (SAP) will act as a key driver in the improvement of services for older people. Leeds Teaching Hospitals NHS Trust (along with the city's social services department, mental health trust and other local agencies) has implemented a form of contact assessment that works in both hospital and community settings. This article describes the principles behind the assessment of older people's needs and the challenges inherent in developing a person-centred framework for assessment. The Leeds example demonstrates that as a result of introducing SAP, improved person-centred information follows the patient on discharge. It is suggested that whilst awaiting the arrival of the NHS IT strategy (or a local interim SAP software solution), a window of opportunity exists for staff to ensure the principles of the SAP are well-established.
