AnnotationThe Community Care (Delayed Discharges etc) Act 2003 placed an obligation on local authorities to reimburse NHS hospital trusts for delayed discharges from hospital. A previous report by CSCI, "Leaving hospital - the price of delays" (October 2004) examined the impact of this policy. In the main, the present report is based on discussions with the same group of older people and their carers who provided evidence for the 2004 report. They were interviewed about their experience of health and social service support since then. The main findings concern: continuity of care; better contingency planning; a focus on rehabilitation; a more flexible role for residential care; problems of day in, day out pressures on carers; and having a genuinely comprehensive, preventive service. The report notes that all places visited had taken further steps to improve the speed and co-ordination of hospital discharge. Very few of the re-admissions seen this time had resulted in a delayed discharge. (RH).
