Annotation | Community matrons have been introduced to work across health and social care, and are taking on case management of people with long-term conditions. Julie Rae, a trained district nurse, became the first community matron in February 2005, when Selby and York Primary Care Trust began a 3-month pilot scheme. The author spent a day with her, to find out how community matrons will fit in with the work of a PCT. (RH). |
