Annotation | The Evercare Programme was originally developed in 1987 in the US, where it now covers 71,000 people. This programme of case management for frail older patients was introduced into nine primary care trusts (PCTs) in England in 2003. Patients and carers have expressed great satisfaction with the programme, while nurses have reported that their skills have been enhanced, and general practitioners (GPs) generally thought that the care of patients was improved and their workload lighted. The aims of this assessment were to evaluate the programme from several perspectives: effectiveness of a change programme working across a whole system; impact of the programme on patient care and on clinical staff; and effectiveness of the programme in changing care pathways and reducing avoidable hospital admissions. Results are structured according to Evercare's five core principles: applying an individualised, whole-person approach to care; using primary care as the central organising force for health care across the continuum; providing care in the least invasive manner; avoiding adverse effects of medication and polypharmacy; and using data to strengthen decision-making.
