Annotation | Results presented in Volume 1 were organised around the five Evercare principles. The first part of this appendix provides a more detailed presentation of these principles. The remaining eleven appendices provide additional detailed information for the primary care trusts (PCTs) involved: Airedale, Halton, Walsall, Bexley, Luton, Wandsworth, Bristol North, Bristol South & West, and South Gloucestershire. Appendices cover the detail results for general practice system, acute trusts and medication; survey results on experience of advanced primary nurses (APNs), patient and carer satisfaction, programme and clinical leaders, general practitioners; PCT executives; APN operational database; success stories sampling; and Evercare Programme population characteristics.
