Annotation | The National Confidential Enquiry into Perioperative Deaths (NCEPOD) is now under the umbrella of the National Patient Safety Agency (NPSA) as an independent confidential enquiry whose main aim is to improve the quality and safety of patient care. This study is based on all patients aged 16+ admitted to general intensive care units (ICUs) in England, Wales and Northern Ireland during June 2003 (a sample of around 6,000). Each patient was monitored and results recorded during the study period on whether a patient died, was transferred to a similar facility, was downgraded to Level 2 care, was discharged from the ICU, or was still alive in the ICU. This report presents findings and recommendations on: pre-ICU care; patient observations and review criteria; the referral process; ICU admission processes; patients who died; outreach; quality of medical records and audit; and pathology.
