Annotation | 25 primary care trusts (PCTs) and their local authority and acute and mental health service partners across Yorkshire and Lincolnshire have been involved in an extensive period of consultancy. The emphasis has been on sharing experience of implementing national policies and on learning together, which has facilitated co-evolution within whole systems approaches to planning intermediate care.
