Annotation | Some older people require a high level of ongoing health and personal care after an acute hospital stay, which they may receive in specialised accommodation such as a nursing home or community hospital ward. In this article, the authors describe some of the problems faced by patients and staff, and offer solutions from the experience of an NHS continuing care ward, including patient transfer and ethical aspects of continuing care.
