AnnotationEvercare is a model of care for frail older people, containing elements of nurse-led assessment and intensive case management. An independent evaluation of Evercare is being carried out by a team from the National Primary Care Research and Development Centre (NPCRDC), Universities of Manchester and York, the Manchester Business School, and the Sheffield Institute for Studies into Ageing (SISA), University of Sheffield. This summary presents an outline of the interim findings of the evaluation: background, methods and policy implications. Among the findings discussed is whether Evercare has reduced hospital admissions. The evaluation will be completed in early 2006. (RH).
