Annotation | Case management is being developed across the National Health Service (NHS) in England as a tool for improving care to patients with long-term conditions. Case management comprises the following six core elements, any of which may be used in a particular setting: case finding or screening; assessment; care planning; implementation or management; monitoring; and review. This review of published research on case management aims to: describe methods of patient selection; evaluate the impact of case management on health care utilisation and patient health; and review the reported cost-effectiveness of case management. The findings are discussed in the context of current NHS policy. (RH). |
