Annotation | Standard four of the National Service Framework for Older People (NSF) relates to general hospital care. The development of multidisciplinary teams is suggested as a way of implementing the standard. The author describes an example of this approach, an older persons' outreach and support team (OPOST). Among its aims are to improve the discharge planning process, and to identify and maximise rehabilitation potential whenever possible. The article covers service evaluation, organisational issues, multidisciplinary team working, and delivering person-centred care. (RH). |
