Annotation | The Evercare Programme was developed in the United States, with the aim of improving the quality of life for vulnerable older people. The Programme has been piloted in nine Primary Care Trusts (PCTs) in England. This interim report has been produced mid-way through the Evercare project's 17-month implementation phase, on the basis of which, the Department of Health (DH) has decided to expand the Programme in England. Evercare is an affiliate of the UnitedHealth Group. (RH). |
