This toolkit focuses on the practical steps that health and social care professionals can take to improve discharge. Changing the way in which discharge occurs for the 80% of patients who are classified as simple discharges will have a major impact on effective use of bed capacity and improve patient experience. The toolkit includes a 10-step guide, factsheets and case studies with information about how others have made changes. (RH).
