Annotation | In contrast to other commentators, the authors of this article believe that the introduction of the Community Care (Delayed Discharges etc) Act 2003 has provided the catalyst for some helpful work to reduce the effect of delayed discharge. However, they do recognise that problems remain in the system that will not be resolved by reimbursements. Their article considers the development of policies to implement the Act in one locality, Nottingham. They suggest three main elements to an effective method of hospital discharge: good quality information passed from health to social services; an efficient system of assessment and care planning within social services; and a network of community care services to support discharge.
