Cancer in older patients

AnnotationWith increasing life expectancy, older patients are becoming the dominant patient group in oncology clinics. Many older cancer patients do want effective cancer treatment, but the potential serious toxicity and unpredictable outcomes can engender a degree of reluctance on the part of the physician. This literature review covers: assessment; the biology of cancer; diagnosis and screening; management of cancer in the older patient; surgery and anaesthesia; chemotherapy; radiotherapy; and palliative care. While many studies have been published that show standard cancer treatments can be applied to patients in later life, most recruit mainly those who are fit, and so provide an unrealistic picture of what could be achieved.
