Annotation | The impetus to develop innovative intermediate care services on the Isle of Wight has been driven by the fact that the island has the second largest population of older people in the whole of the UK. The authors describe how professionals from the NHS Trust, the Primary Care Trust (PCT) and the social services department (SSD) have used a whole systems approach to develop successful day care, respite and rehabilitation centres.
