Annotation"Shared care" means systematic, carefully planned and sustainable collaboration between general practitioners (GPs) and hospital specialists in the medical management of specific conditions. As a milestone in the implementation of the National Service Framework for Older People (NSF), all primary care trusts (PCTs) must have "shared care" protocols for dementia in place by 1 April 2004. The authors suggest a model shared care protocol, and outline the next steps that PCTs can take to improve dementia care in the community. (RH).
