Annotation"Intermediate care" has been held up as a way forward for health and social care for older people. The book is divided into four parts. Initial discussion centres on the concept of intermediate care and the motives for its development. Part 2 shows how older people are one of the main client groups to benefit from intermediate care, demonstrating how it can be best developed to serve their needs. Part 3 discusses the practical aspects of planning, developing, monitoring and evaluating care services. Included are chapters on care pathways, the Single Assessment Process (SAP), and the role of caregivers in home care. Part 4 focuses on the delivery of care to older people in areas such as person-centred care, rehabilitation, and discharge planning, and with an emphasis on the importance of staff development and training.
