AnnotationThis is the first annual report of the Health and Social Care Change Agent Team (CAT), and summarises key messages and learning points that have emerged from its work so far. Implementing the National Service Framework for Older People (NSF) has been the main theme underpinning the Team's work. Examples of good practice illustrate work on: whole system working; discharge planning; reimbursement (for delayed discharges); better information; avoiding unnecessary hospital admissions; intermediate care; housing as an alternative to hospital and long-term care; mental health services for older people; and commissioning non-acute services for older people.
