AnnotationPilot projects run in conjunction with the London Older People's Service Development Programme have been helping London boroughs to meet the April 2004 deadline for implementing the single assessment process (SAP) under the National Service Framework for Older People (NSF). This article describes one of the 25 borough-based pilot projects, a care co-ordination service (CCS) in Brent. The scheme identifies older people at risk - high-resource users aged 75+ - and co-ordinates work across care-team boundaries. Hospital admissions among older people helped in the project have dropped by 47%. An evaluation of the programme is being undertaken by the Institute for Applied Health and Social Policy. (KJ/RH).
