Annotation | Concerns regarding the adequacy of care for people with severe mental disorders in the UK have concentrated almost entirely on younger people. The authors investigated the prevalence of schizophrenia and related disorders in older people (aged 65) from a defined catchment area - Lanarkshire, Scotland - using the Cardinal Needs Schedule to assess health and social care needs. The 1-year prevalence of schizophrenia and related disorders was 4.44 per 1,000 of the population at risk. There were high levels of unmet need for many patients, including those in National Health Service (NHS) continuing care beds. Many needs were identified, for which all of the existing skills of local health and social care professionals could be used. The investigation raises serious concerns about standards of hospital and community care for older patients with schizophrenia. The findings may be unique, reflecting long-standing problems with a particularly hard-pressed part of the NHS. However, it is not known whether a similar situation exists in other parts of the UK.
