Annotation | An innovative scheme to improve the care of older people with mental health difficulties by integrating inpatient and community services has won a team from Cumbria a Nursing Times award. This article reports on the work of North Cumbria Mental Health and Learning Disabilities NHS Trust's Care Home Education Support Service (CHESS). Having identified problems for care home staff, the CHESS team introduced changes in training, collaboration, involvement of users and carers, and access to folloW-up care. (RH). |
