AnnotationSome of the specific needs and issues facing older homeless people at the point of discharge from hospital are outlined in this summary of findings by the Older Homelessness Development Project (OHDP). The report uses seven models of service provision to argue that health and social care agencies must work in close partnership with housing providers and homelessness agencies.
