AnnotationWhen older homeless people leave hospital, all too frequently arrangements for their accommodation are inadequate or inappropriate. Based on research by the Older Homelessness Development Project (OHDP), this report urges health and social care agencies to work closely with housing and homelessness organisations to ensure that older homeless people are properly linked to community services. The report comprises a literature and policy review; findings on health, housing and hospital contact among older homeless people; and findings from interviews with hospital social workers and staff involved in discharge. Seven models of service provision are reviewed: Primary Care for Homeless People (PCHP), London; Three Boroughs Primary Health Care Team (in the London Boroughs of Lambeth, Southwark and Lewisham); Creative Support, Edale Unit, Manchester; Newport Action for the Single Homeless (NASH), Newport, Wales; Wytham Hall, Maida Vale, London; Manchester Housing protocol for hospital discharge; and Salford local authority /health authority temporary sheltered scheme.
