AnnotationThe NHS Plan proposed a range of intermediate care services designed to bridge the gap between hospital and home. Subsequently, intermediate care features as one of the eight core standards of the National Service Framework for Older People (NSF). Intermediate care aims to help people recover and regain independence more quickly; bring about swifter hospital discharge when people are ready to leave; and avoid unnecessary long-term care. Against a backdrop of a wide range of definitions and different models, the author considers what is meant by intermediate care, and reviews recent government policy. She explores what care older people themselves want, and their views on the outcomes of their care, which need to be taken into account when planning the future of intermediate care. (RH).
