AnnotationThe purpose of the Joint Review is to provide an objective assessment of how well local people are being served by their social services, and to make a constructive contribution to the further improvement of services provided. The Review finds that some but not all people in Hillingdon are well served by their social services, and prospects for improvement are uncertain. However, the Authority has begun to develop processes to empower users and carers through their involvement in care management processes, carers' assessments, and Direct Payments. However, community-based services for older people are underdeveloped: the patterns of service tend to foster dependence rather than maximising independence. An example of good practice is the Whitby Dene intermediate care centre, a joint primary care trust (PCT) and Social Services unit that provides rehabilitation after hospital admission. Priorities for action identified by the Review include: sorting out the care management processes that are not working well; and developing more community-based services. (RH).
