Independent multidisciplinary review before entry into institutional care

AnnotationIn 1997, Gloucestershire had an average proportion of older people publicly funded in care, but a high percentage of these placements in nursing rather than residential care. This suggested that assessment and placement were unsatisfactory. The desire both to minimise the institutionalisation of older people, and to ensure that care appropriate to the needs of the client was provided, led to to the creation of the Countywide Assessment Team (CWAT) in Gloucestershire. The team's primary purpose was to ensure that multidisciplinary assessment and adequate rehabilitation had been carried out before funded care was granted by social services, and to audit the quality of those assessments. This article outlines the methods used and the results after 18 months. The introduction of an independent multidisciplinary assessment panel has brought consistency to the decisions and categorisation of care for state-funded entrants to institutional care across the county. It has also been possible to advise alternatives to admission to care, such as intermediate care. The team's work is also seen as an important way of auditing the quality of assessment. (RH).
