AnnotationHartlepool became a unitary authority at the dissolution of Cleveland County Council in 1996. The purpose of the Joint Review is to provide an objective assessment of how well local people are being served by their social services, and to make a constructive contribution towards the further improvement of the services provided. Overall, most are well served in Hartlepool. There are some nationally recognised services for vulnerable adults, such as the Multi-link Team which provides a range of intermediate care, and good hospital discharge support services. Users and carers felt positive about social workers and home care staff. The report notes that the Authority inherited a high level of admissions of older people to residential and nursing homes, though this has decreased markedly with more people being supported at home with intensive service packages. Priorities for action identified by the Review include: involving users and carers more systematically in planning and service provision; and developing formal and robust joint working relationships with the primary care trust (PCT) and other partners.
