Annotation | More than 60% of older people have at least one admission to hospital in the last year of life, with the majority having multiple admissions. The authors report that where there is a diverse minority ethnic population - as in Bankstown, New South Wales, Australia - there were multiple admissions and utilisation of hospital beds for older people in the last year of life. Stroke, as well as stroke and fracture, were strongly correlated to care plan and not for resuscitation orders. Of the 110 patients in this study, only 8 had an advanced care plan documented prior to last admission. However, of the advanced care plans documented, 7 of the 8 were generally clearly written and followed through appropriately. (RH). |
