Care plans, pathways, people a personalised approach to care | Author(s) | Joanne Schofield |

Annotation | Through the example of care for Mr X, the author describes how the development and use of Personalised Care Pathways (PCPs) can help professionals to treat "patients" with dementia as people. To develop a PCP, staff try out and evaluate the effect of different interventions on the individual, and thereby devise a thorough, detailed plan of care that incorporates small yet significant details that acknowledge the patient's needs. The benefit of care plans used previously - which may be too standardised - is questioned. The general observation is that use of PCPs can greatly improve quality of life, and significantly reduce the frustrations and feelings of powerlessness in the person with dementia. (RH). |
