Annotation | Herefordshire was established as a unitary authority in 1998. The purpose of the Joint Review is to provide an objective assessment of how well local people are being served by their social services, and to make a constructive contribution towards further improvement of the services provided. It finds that the Authority is serving some people well, with promising prospects for improvement. For older people, there is insufficient intermediate care and home support provision to assist them to remain at home or to return home from hospital. However, a Homeward Bound re-ablement team in a residential care home, which offers an intensive service for older people discharged from hospital, is given as an example of good practice. Priorities for action identified by the Review include: making sure people understand what they can expect; improving the standard and consistency of assessment, care planning and reviewing; and building on recent positive developments to support people to maintain their independence, participate in the community, and enjoy a better quality of life.
