Annotation | The Well-Being project, a scheme offering joint health and social care assessment to people aged 75+ in the London Borough of Camden identified more than a quarter of them as needing its services. This article by a group of researchers from the Royal Free and University College Medical School notes that using general practitioners (GPs) to identify the target population proved inefficient, as almost half of those contacted were no longer at the same address. Assessments, conducted by a community nurse and a social welfare officer, often involved two visits. Given such problems, the article suggests that the project has provided valuable learning for implementing targets in the National Service Framework for older people (NSF), including single assessment, falls prevention, and reviewing medication.
