Annotation | The purpose of the Joint Review is to provide an objective assessment of how well local people are being served by their social services, and to make a constructive contribution towards the further improvement of the service provided. It finds that most people in Hackney are well served by social services, and the prospects of services improving in the future are promising. A range of good quality services that protect vulnerable people and promotes independence is being developed. Joint working with NHS staff has resulted in good quality and integrated services supporting people to remain in their own home, and ensuring timely discharges from hospital. Examples of good practice cited in the report include rehabilitation services for older people developed jointly with the Primary Care Trust (PCT) and Homerton Hospital; and teleconferencing to enable the housebound to participate in groups such as the City and Hackney Older People's Reference Group. Priorities for action include: continuing to improve the experience of service users and carers; building on the successful partnerships with health; and developing preventative services. (RH). |
