Annotation | Reading is one of six unitary authorities created in 1998 from Berkshire. The purpose of the Joint Review is to provide an objective assessment of how well local people are being served by their social services, and to make a constructive contribution towards the further improvement of the service provided. Some people are served well by Reading, and prospects for improvement are excellent. The Department has chosen to locate and align community care teams with GP practices as a strategy for integrated provision of services; this includes a jointly funded Intermediate Care Co-ordinator and an intensive Domiciliary Care Support Team. Examples of good practice include: West Berkshire joint protocol to facilitate timely discharges for people needing long-term care; residential accommodation with enhanced care; and the West of Berkshire Partnership Board, which has overseen initiatives such as a Performance Improvement Plan in relation to delayed discharges. Priorities for action identified by the Review include: addressing weaknesses in social work practice; consolidating and developing partnership arrangements; and implementation of a workforce development plan.
