Annotation | 130 clients from five home care agencies in Perth, Western Australia agreed to evaluate Version 1.0 of the Primary Assessment Form (PAF). The PAF included assessment items from the Home and Community Care (HACC) Minimum Data Set (MDS), instrumental activities of daily living (IADLs), activities of daily living (ADLs), emotional health, and informal support systems. Evaluation of the different modes of assessment revealed substantial agreement for almost all demographic, IADL and emotional health data items. Qualitative feedback from test assessors indicated positive industry response to the instrument. Based on the success of the preliminary trial, a field test of the instrument across a more diverse state-wide setting and exploration of its ability to support a community care classification has commenced.
