AnnotationConwy became a unitary authority in 1996, from parts of Gwynedd and Clwyd. The purpose of the Joint Review is to provide an objective assessment of how well local people are being served by their social services, and to make a constructive contribution towards further improvement of the services provided. The review found that some people in Conwy were served well by social services, with most people valuing them. but weaknesses prevent all people being served well. Examples of good practice identified by the Review include: ECAT (Elderly Care and Assessment Team) and CMART (Conwy Multi-Agency Response Team), which provide good assessment and avoid delayed discharge of patients from hospital; and assisting people resident in a care home to fund their placements through benefits. Priorities for action identified by the Review include: keeping service users at the heart of service delivery and planning, by focusing on outcomes; offering strong direction and working jointly with people and agencies; and performance and organisation concerns.
