AnnotationA wide range of examples of good practice was found in this SHAS review of services for older people in NHS Forth Valley: training; palliative care services; advocacy services; quality of life groups in community hospitals; training on nutrition in the Primary Care Trust (PCT); and rapid assessment and outreach services. The review also recommends that NHS Trusts and local authorities in the Forth Valley area (Stirling, Falkirk and Clackmannan) should ensure that the strategy for older people has an implementation plan to deal with issues including: a reduction in the number of delayed discharges; the future of community hospitals; the number and siting of NHS continuing care beds; and a redesign of acute and rehabilitation services.
