Annotation | The quality improvement strategies that have been adopted in the US and the UK are doomed to fail, mainly because of too much reliance on relatively narrow, "single-level" changes that the authors say have proved largely unsuccessful. Their summary of an article in Milbank Quarterly (vol 79, no 2, June 2001) recommends four levels of change: individual (health care professional), the team, the overall organisation, and the larger system or environment. This multilevel approach must recognise the importance of: leadership at all levels; an organisational culture that supports learning throughout the care process; emphasis on the development of effective teams; and greater use of information technologies for continuous improvement work and external accountability.
