Annotation | Older people are the most vulnerable members of society and often have complex, multidisciplinary needs. The authors report on a city-wide primary care initiative in Nottingham that provides comprehensive care for older people in nursing homes. Such support enables nursing home staff to manage an older person's care in a way that the individual is no longer isolated. (RH). |
