Annotation | The concept of care pathways emerged in the US in the 1980s, as a way of planning and structuring a patient's "journey" through a care episode from admission to hospital to discharge and beyond. Constructing care pathways presents a challenge in areas where multidisciplinary teams already feel burdened with paperwork. However doing so successfully can be very rewarding for staff, patients and carers. (RH). |
