AnnotationIn a case series of 84 seriously ill American home health patients (median age 75, aged 37 to 94), most (82%) preferred to die at at home, and virtually all were willing to participate in a home-based advance care planning (ACP) process. Facilitating ACP for such patients and their families was associated with end-of-life care at home. Use of hospice services was common following ACP in this population. (RH).
