Annotation | Older people's experiences of an intermediate health care scheme, which seeks to prevent inappropriate admission to hospital and to facilitate early discharge, are described. Mapping the relationship of intermediate care to the whole continuum of care, it is suggested that the scheme treads an increasingly fine line, acting as a compensatory mechanism for deficiencies in the mainstream system, whilst also being an unwelcome addition to it, with an extra tier of bureaucracy and a further barrier to seamless services. The main implication for cross-disciplinary working is that without comprehensive training in assessment and care management, the problems endemic in the wider mainstream system, such as inadequate assessment, mirror themselves in intermediate care.
