Annotation | Based on a survey of 23 NHS Acute Hospital Trusts in North West England, this research found that hospital discharge policies acknowledge the involvement of carers, but do not have practical measures in place to identify and support carers. This research is part of the Carers UK (previously the Carers National Association) health campaign, Carers' Health Matters, and supplements earlier research reported in "You can take him home now". It reports key findings and provides a good practice checklist on: choice about caring; identifying the care; planning for discharge; carers' assessments; support at home; and consultation and monitoring.
