Annotation | Neath Port Talbot was formed in April 1996 from parts of former West Glamorgan. The purpose of the Joint Review is to provide an objective assessment of how well people in Neath Port Talbot are being served by their social services. The review notes improvements in hospital discharge arrangements, with few patients delayed in hospital to have their needs assessed. A comparatively high number of older people receive forms of "institutional" care. Examples of good practice include the role of the Re-ablement Team in enabling older people to remain at home; resource and needs mapping in relation to services for older people; and "good care" training for residential home and day care staff. The Joint Review concludes that the Authority's strategic plan to "remodel" older people's services will need to be accompanied by further changes in assessment and care management practice.
