Annotation | Changes are required to achieve a higher standard of care for older people. Existing resources could be used more effectively, but new resources will be needed to meet increases in workload. General practitioners (GPs) need to show that managing a person at home or in a nursing home, as an alternative to hospital, does not provide inferior care. Case management and integrated care could be ways of confidently managing people in the community. Primary care groups (PCGs) could play a key role in implementing this. (RH). |
