AnnotationA pilot project in Hammersmith and Fulham to assess the feasibility and potential benefits of targeted screening and home assessment is reported. The Keep Well at Home method comprises an initial screening, comprehensive assessment, onward referral and regular telephone monitoring for those aged 75 and over at risk of functional decline. Using validated tools, this preventive approach links health and social care, and is proving feasible in practice. (RH).
