A comparison of five clock scoring methods using ROC (receiving operating characteristic) curve analysis

AnnotationReferrals, mean age 78.2, to a geriatric outpatient clinic in Sydney, had the clock drawing test at the beginning each appointment with an observer blinded to existing medical history and was subsequently assessed by a geriatrician. Each clock was scored according to methods of Mendez, Shulman, Sunderland, Watson and Wolf-Klein, and evaluated for reliability and predictive accuracy using receiver operating characteristic (ROC) curve analysis. The Shulman and Mendez methods predicted dementia more accurately than the other methods. While substantial differences among the clock scoring methods were evident in the sample, the accuracy of each was modest at best. Unless further studies in relevant settings suggest otherwise, the authors caution on use of clock drawing alone to screen for dementia. (RH).
