AnnotationThe Government regards the divide between health services and and social services as a barrier to "joined up" access to services. One of the aims of the Health and Social Care Bill is a framework for setting up Care Trusts, which would be able to work across organisational boundaries. This document outlines possible models and client groups for Care Trusts. It does not represent any final sort of policy, but considers ways in which Care Trust policy is being designed and developed - which is being done in co-operation with a wide number of groups and organisations.
