Acute stroke in very old people
 — clinical features and predictors of in-hospital mortality

AnnotationData for 2,000 stroke patients - 262 very old (aged 85+) and 1738 patients aged less than 85 - were compared at a Barcelona hospital. Predictors of in-hospital mortality based on clinical and neuro-imaging variables were recorded within 48 hours of stroke onset, and outcome variables (medical complications developing during hospitalisation) were assessed. Very old patients with acute stroke showed a differential clinical profile: their in-hospital mortality was 27%, compared to 13.5% for the under 85 group. Clinical and neuro-imaging factors indicative of the severity of stroke and that were available at the time of initial diagnosis and at the time of development of cardiac and respiratory complications showed a predominant influence on in-hospital mortality, and may help clinicians to evaluate prognosis more accurately.
