AnnotationDementia with Lewy body (DLB) has been reported to account for 10-15% of all cases of dementia, and is associated with an increase in psychopathology. The authors searched the database of the University of California, San Diago SOCARE (Seniors Only Care) outpatients programme. There were 26 autopsy-confirmed cases of DLB. These were matched individually with 26 autopsy-confirmed cases of "pure" Alzheimer's disease (AD) on gender, ethnicity, and Mini-Mental State Examination (MMSE) score at baseline evaluation. Comparison of the two groups on psychopathological measures and possible risk factors indicated that five psychiatric symptoms - hallucinations, delusions, anxiety, anhedonia, and loss of energy - were significantly more common in DLB than AD patients. DLB patients were significantly younger at initial evaluation and death compared to AD patients, but there was no difference in age at onset of dementia, level of education, or family or past history of any major neuropsychiatric disorder, prescription of psychotropic medication, or sensory impairment.
